
What is
the Community Based Outpatient Clinic (CBOC) Program?
VA North Texas Health Care System has
developed an innovative CBOC program to provide health maintenance and
preventive health care within 30 miles or 30 minutes of a veteran's
home. CBOCs reduce travel time and shorten waiting times for
veterans needing outpatient care.
Who is
eligible to participate in the CBOC Program?
Any veteran in priority
groups 1 - 8a, 8c is eligible to enroll.
What services
are available at CBOCs?
Primary Care, including:
- routine physical examination
- laboratory testing
- prescriptions
- immunizations
- preventive health maintenance
Is specialty
care available through a CBOC?
Once a CBOC provider determines the need
for specialty care, a consult is submitted to VANTHCS. Referral with the specialty clinic is
coordinated with the nearest VANTHCS facility, and the
patient is notified of appointment date and time.

enroll in the CBOC program?
Veterans interested in enrolling in the

Once a veteran has been found to be
eligible for enrollment, he/she will be
added to the CBOC registry at the beginning of the next month. The
contractor for that registry will send the patient a
welcome letter which includes the phone number and address of the clinic nearest to the
patient's
home address.

Veterans new to the VA North Texas
Health Care System will need to furnish a copy of DD
Form 214 Certificate of Release or Discharge From Active Duty (DD-214).
If the veteran does not have a DD-214, a
local veterans service officer may
assist them in obtaining one.
